Our qualified clinicians are certified ADP Authorizers. We assess cognitive and physical abilities and match device selection to your goals, your environment, and your day-to-day function.

We can assess and prescribe wheelchairs, seating and cushions, walkers, and canes, with a respectful, professional process designed to help you regain and maintain optimal mobility.

How the ADP Process Works

If you believe you may qualify for a mobility device funded through the Ministry of Health Assistive Devices Program (ADP), contact ACHIEVA HEALTH to schedule an assessment with an Occupational Therapist or Physiotherapist ADP Authorizer.

We screen eligibility, explain the process, and coordinate the comprehensive assessment with your chosen vendor to ensure the right device fit, configuration, and function.

Eligibility Screening

Your ADP Authorizer screens you to determine whether you meet ADP criteria and reviews the steps and consent.

Device Assessment + Vendor Coordination

A joint assessment is scheduled with your selected vendor to confirm the best device (wheelchair, walker, cane, seating/cushion) for your needs.

Ordering + Follow-Up

We complete the paperwork, coordinate ordering, and support a trial period with reassessment to confirm the device is appropriate.

Canes

Promote upright posture with eyes forward, walk slowly, stand up safely without pulling on the device, and monitor brakes, wheels, and grips for wear and tear.

Proper use of walking aids can reduce falls risk and improve confidence. In-home care allows us to coach safe movement strategies in your actual home environment.

Funding and Payment Options

In-home services are typically paid privately, either out-of-pocket or through private insurance coverage. We provide documentation to support insurance reimbursement when applicable.

For eligible mobility devices through ADP, the Ministry typically covers 75% of the cost and you are responsible for the remaining 25%.
