What Is a Government-Funded Episode of Care (EOC)?
An Episode of Care (EOC) is a government-funded physiotherapy program delivered through designated Community Physiotherapy Clinics. ACHIEVA HEALTH - Eglinton participates in this program to support eligible patients.
Patients who meet the eligibility criteria may access an Episode of Care without a physician referral. Care begins with a comprehensive physiotherapy assessment and continues through treatment until physiotherapy goals are met and the patient is discharged.
Who Is Eligible for Publicly Funded Physiotherapy (OHIP)?
You may be eligible for publicly funded physiotherapy services at our Eglinton clinic if you are referred by a physician or nurse practitioner, insured under OHIP, and meet one of the following criteria:
- Aged 65 years or older
- Aged 19 years or younger
- Recently discharged from hospital following outpatient or day surgery
- Eligible under the Ontario Disability Support Program or Ontario Works
What Can I Expect From My Physiotherapy Treatment Plan?

Do I Need a Doctor's Referral?
A doctor's referral is not required to start physiotherapy at our Eglinton location. Government-funded Episodes of Care do not require a physician referral if eligibility criteria are met. However, some private insurance plans may require a referral from a physician or nurse practitioner for reimbursement. Our team can help clarify what is required.
How Many Physiotherapy Visits Will I Need?

For government-funded Episodes of Care, there is no fixed number of visits. Treatment frequency and duration are based on clinical judgment, individual needs, and available funding.
